History:
(Captain America I#221/2) - Responding to a high-priority
summons, Rick Jones rushed into the Avengers' conference room; but he
inadvertently tripped over some cables, fell into a large viewscreen,
and electrocuted himself (!) into unconsciousness.
Rising from the wreckage, Rick found that he was now a super-hero, clad in a colorful costume that was an amalgamation of the four male Avengers, and he also possessed superhuman strength. He decided to call himself "Captain Avenger".
But just then, Baron Zemo and an army of his agents burst in. Zemo fired a ray gun which immobilized the Avengers, leaving only Rick unaffected. With the five Avengers transformed into motionless stone-like statues (see comments), Rick retaliated and fought his way past Zemo's agents, and to Zemo himself.
After destroying the ray gun, Rick grabbed Zemo; but the fugitive Nazi scientist merely laughed--he explained to the horrified Rick that the ray gun was the only way to restore the Avengers to normal, and now they were forever frozen!
In response, the enraged Captain Avenger took a swing at Zemo... but then Rick woke up and came to his senses when he saw Captain America and the other Avengers very much alive, and thus learned the whole experience of his being Captain Avenger had all been just a dream.
As he walked out of the meeting room, the crestfallen Rick realized he'd never be a super-hero.

This 5-page story was set between Avengers I#14 and 15, following their encounter with the Kallusians, and just before the death of the real Baron Heinrich Zemo.
Sheesh, what a crummy story. Go back to DC's Silver Age where you belong, imaginary story!
Captain Avenger's costume consisted of Iron Man's armored boots and leg coverings, Thor's trunks and belt, Captain America's shirt, sleeves, and gloves, and Giant-Man's chest overlay--if the story had lasted a few more pages, it's possible he had a combination of their powers as well.
When the dream Zemo used his ray gun to petrify the dream Avengers, possibly the unconscious Rick was recalling when the real Avengers were transformed into stone statues by the "Medusa" extraterrestrial Vuk of the D'bari race (@ Avengers I#4).
